Can I supply the materials?

Mention this when requesting the estimate. Product suitability, quantity, delivery, missing pieces and responsibility for returns should be settled before the work is booked.

Does replacing a floor include new baseboards?

Only if they are included in the agreed scope. Ask whether trim will be reused or replaced, and whether painting, disposal and floor transitions are included.
